Hospice Medical Social Worker:   Enjoy independence and flexibility in the field to visit patients one on one in their own homes.  

Hours:  Full-time, Days | Monday - Friday and On Call Rotation

Territory:The Social Worker will have a general territory within our service area where they will provide one on one in-home care to our patients.  The territory for this position would be in the East section of the metro.

A few must-haves for this position:

  • Graduate of a Bachelor’s program in social work accredited by the Council on Social Work Education required.  Masters degree in social work preferred.
  • Licensed to practice as a Social Worker in the State of MN.  License is active and in good standing.
  • Minimum of one (1) year's experience in health care.
  • Hospice experience preferred.  
  • Able to travel within geographic area serviced by office. Valid drivers license, and proof of auto insurance is required.

Some functions of what you will do as a Social Worker:

  • Conducts an initial psychosocial assessment of assigned patients/families within five (5) calendar days of admission to contribute to the comprehensive assessment of the patient’s/family’s needs and the Interdisciplinary Group (IDG) plan of care.
  • Provides individual and/or family counseling in response to the mental and emotional status of the patient/family to reduce barriers to achieving the goals of care.
  • Identifies special needs related to religious or cultural diversity and how these impact the implementation of the IDG plan of care.
  • Provides counseling in response to identified anticipatory grief or pathological grief, if so qualified, or identifies alternative resources for such and provides direction to those resources.
  • Provides crisis intervention support to the patient/family, members of a facility caregiver group or the IDG.
  • Participates in the development of the IDG plan of care and attends regularly scheduled IDG meetings.
