Disease Surveillance Investigation and Partner Services

<li>Receive reports of STI's/HIV that require an interview and public health follow-up.</li>
<li>Initiate investigation within 48 hours of case assigned workable contacts.</li>
<li>Interview individuals diagnosed with reportable STI/HIV to identify others at risk for infection.</li>
<li>Identify, locate, notify &amp; refer for examination and or/ treatment sex partners of person infected with a sexually transmitted disease(s) including field visits to locate individuals who are hard to reach.</li>
<li>Effectively utilize telephone calls, text messaging and letters to confidentially locate individuals with a reported STI or HIV diagnosis.</li>
<li>Provide client centered counseling on reducing risks of acquiring STI's in the future.</li>
<li>Coordinate referrals for other services as appropriate.</li>
<li>If appropriate, make appointments for sex partners to be seen at a county health department clinic.</li>
<li>Effectively utilize ORPHEUS database for casework and surveillance</li>
<li>Accurate completion of entries of required data into ORPHEUS</li>
<li>Conduct EISO Orpheus data audit and monitoring.</li>

Community Outreach Testing Sites
<ul>
<li>Utilizes a variety of communication strategies to engage the target population.</li>
<li>Assists clients with forms (consent, HIPAA, demographics, history, ROI) as needed.</li>
<li>Perform Rapid HIV /syphillis testing.</li>
<li>Assists RN to collect lab specimens, blood draw, urine, swabs.</li>
<li>Check temperatures every hour for Rapid HIV/Syphillis test kits and Rapid HIV/ syphillis test controls.</li>
<li>Help assure that outreach materials and supplies are stocked and ready for outreach events.</li>
<li>Helps ensure that forms and brochures are up to date. </li>
<li>May assist in driving the mobile outreach van.</li>
<li>Assist with cleaning and maintenace of mobile outreach van.</li>
<li>Drive to outreach service locations, attend meetings, participate in health fairs, etc.</li>

<li>Implement activities planned by program coordinators for STI/HIV/EISO Clinic services to develop, implement, and maintain a data driven, culturally appropriate community outreach plan to at risk populations and the agencies/providers serving them.</li>
<li>Participate in the development and implementation of new models of health promotion including but not limited to effective behavioral interventions for community and individual behavior change.</li>
<li>Shares mission and values with collaborating organizations, the community, and advocates for needed services and future direction of services.</li>
<li>Stays current on HIV prevention and interventions; meet performance standards and deadlines set forth by county program and partnering agencies regarding quantity, timeliness, and quality works, maintain clinical records in accordance with Federal, State, County Health, and program requirements under the direction of the facilities clinical supervisor.</li>

EXPERIENCE AND TRAINING

Bilingual Public Health Worker 3 - Health and Human Services

Marion County Health and Human Services is seeking a compassionate, energetic, and flexible Program Therapist (Mental Health Therapist 2) to work at Horizon House Residential Treatment Facility. The Program Therapist will work collaboratively within an interdisciplinary team to provide Person-Centered mental health treatment services in a 24-hour residential clinical setting.
This individual will be serving a caseload of up to 9 adult residents with serious and persistent mental illness and/or chemical dependency. Ideal candidates will have direct or related experience working in a therapeutic milieu environment, facilitating therapeutic groups, implementing/writing treatment plans, and managing doctor's orders.
The candidate will collaborate with internal and external resources to ensure the best possible treatment for residents. We actively engage our residents in treatment related activities by promoting community integration, and a recovery model approach that emphasizes independent skill development (e.g. cooking, budgeting, shopping, cleaning, nutrition, medications, social skills, communication).
Schedule is standard M-F, 0830-1700 with some flexibility based on department need. The program therapist will also maintain clinical documentation, and transport clients as needed - ODL is required.

Completes and updates an individual service plan. Obtains and maintains competence in all screening and assessment tools required by the county. <li>Provides level of care determinations and eligibility reviews for youth being referred to the Wraparound Program. Presents cases at the System of Care Review Committee, which include eligibility recommendations.</li>
<li>Participates in individual service plan development in collaboration with the treatment team and other community providers. Ensures the collaborative development, periodic review and revision of an individual service plan. Provides service coordination to assist families in accessing formal and informal supports and services; facilitating communication between the family, natural supports, community resources, and involved child-serving providers and agencies; participates in team meetings at which strengths and needs are identified and safety planning occurs; and provides for continuity of care by creating linkages to, and managing transitions between levels of care, and transitions of older youth to the adult service system.</li>
<li>Completes required documentation and documents services to meet contract, administrative rule, and billing requirements and maintains acceptable level of productivity for these requirements. Expectation is that employee will strive for 60% of work hours on average to be billable hours with a minimum of 50% billable hours on average each month.</li>

Response Level 2: In the event of an emergency, employee makes a reasonable effort to report to work. Employee may be required to perform duties outside of their regularly assigned duties. Requirements for the Position
EXPERIENCE AND TRAINING
<li>Possession of a Master's degree in Psychology, Social Work, Recreational Therapy, Music Therapy, Art Therapy or a behavioral science field, or possession of a bachelor's degree in Nursing and licensed by the State of Oregon, or possession of a bachelor's degree in Occupational Therapy and licensed by the State of Oregon; OR second year graduate students waivered as QMHP's by the Oregon Health Authority.</li>
<li>Mental Health Therapist 2 Only: In addition to the educational requirements above, the Mental Health Therapist 2 must have at least one year of full-time, post-Master's degree experience in the behavioral health field.</li>
PREFERENCES Preference will be awarded to candidates who are LCSW, CSWA or eligible for LCSW. Qualified candidates will be eligible for licensure supervision if hired.
AUTO-PROMOTE AND SPECIAL CONDITIONS OF CLASSIFICATION Individuals hired as a Mental Health Specialist 1 will auto-promote, as noted in Article 4 in the Marion County personnel rules, to the classification of Mental Health Specialist 2 upon satisfactorily completing one-year, post master's degree, of service as a Mental Health Specialist 1. (One-year of work experience is equal to 2080 hours).
SPECIAL REQUIREMENTS

<li>Must be either 1) licensed by the Oregon Board of Licensed Professional Counselors, 2) licensed by the Oregon Board of Licensed Social Workers or 3) credentialed or registered through the Mental Health and Addiction Counseling Board of Oregon (MHACBO) as a Qualified Mental Health Professional (QMHP) within the first 30 days of employment. Applicants may be required to pass a practical knowledge and skills assessment as part of the interview process.</li>
<li>Must possess, or obtain within 60 days of hire, CANS certification (05 &amp; 6-20).</li>
<li>This position may be subject to the following: Must not be excluded from participation in the federal health care programs (Medicaid, Medicare and other federally funded programs that provide health benefits); AND must not be excluded from participating in federal procurement (Federal Acquisition Regulation) and non-procurement activities (Executive Order No. 12549).</li>
<li>Must possess a current driver's license in the applicant's state of residence and an acceptable driving history. Marion County will obtain a copy of the driving record for all qualified applicants from Driver and Motor Vehicle Services and review the driving record according to the Marion County policy and procedure for Driving on County Business. PHYSICAL REQUIREMENTS Operates a motor vehicle; sees using depth perception; sits; stands; moves about the work area; climbs 1 floor of stairs; lifts, pushes, pulls, and carries 5 lbs.; moves carts weighing up to 5 lbs.; operates a keyboard; speaks with a clear and audible voice; reads a 12-pt. font; hears a normal speech level.
