Registered Nurse (RN) Home Care

Marion County Health &amp; Human Services is looking for a qualified individual to work 50% within the Marion County Correctional Facility to provide services to Adults in Custody (AIC) and 50% as a Collaborative Assessment and Management of Suicide (CAMS) Clinician. While in the jail, this position provides crisis counseling and resource/referral information for AIC's. It requires a creative individual who can think outside the box and work compassionately with AIC's as well as the jail staff to provide the best possible outcomes for the individuals we serve. As a CAMS Clinician, this position will provide individuals experiencing suicidal ideation with a framework for providing expedient outpatient services focusing on what is driving the suicidal thoughts.
The person in this position must be able to work independently, problem solve, and be proactive when engaging the individual. We offer the following recruitment bonus, prorated based on FTE:

<li>$2,000 bonus for employees new to Marion County starting on January 1, 2024, through June 30, 2025. Bonus will be included in employee's first paycheck.</li>

SUPERVISION RECEIVED Work under a supervisor who assigns work establishes goals and reviews the results for overall effectiveness.
SUPERVISION EXERCISED Primary supervision of employees is not a responsibility of this classification; however, positions may act as lead workers as defined in the Marion County Personnel Rules.
Typical Duties - Duties include, but are not limited to the following

<li>Provide intensive case coordination, including triage, crisis intervention and referral to jail psychiatric prescriber as appropriate for consumers who are in jail. Provide case management and assessments in consultation and in coordination with jail/medical staff, probation and parole officers, psychiatric crisis center (PCC), pre-commitment, contract staff and adult behavioral health. Facilitate connection with community mental health provider/agency and referral back to treatment. Travel to community agencies and trainings as needed.</li>
<li>Provide triage and functional assessments of mental health, addiction and violence risk issues to adults in custody (AIC) that include a provisional multi-axial diagnosis and interventions as well as long term and short-term treatment and intervention recommendations that promote victim/community safety and continuity of care. Collaborate with family members and significant others as appropriate. Coordinate care for those individuals in jail who are enrolled with community agencies. Participate in the court and team meetings as indicated. </li>
<li>Drive a motor vehicle to attend meetings and staff meetings throughout county agencies.</li>
<li>Provide traditional counseling services as appropriate.</li>
<li>CAMS Brief Counseling for clients on limited, as needed basis.
<ul>
<li>Counseling to CAMS Clients:Provide counseling to individuals referred to CAMS due to suicidal issues. Intervene with both children and adults presenting problems covering the full range of personal needs and include thoughts/plans or violence toward others, and major psychiatric illness. Because of the wide spectrum of presenting problems, it is necessary to have a comprehensive knowledge of psychopathology and to be able to draw upon a wide range of therapeutic strategies in counseling.</li>
<li>Assessment of Mental Status: Perform independent assessment of mental status, diagnosis, level of functioning, dangerousness, and specific needs for services.</li>
<li>Planning Development: Develop plan with client/family to address immediate needs and plans to obtain services. If client is unwilling to seek services, make decisions regarding involuntary commitment based on dangerousness and ability to care for self.</li>
<li>Community Coordination: Maintain current knowledge of, and contact with, community agencies in order to make referrals appropriately. See that clients gain access to the services they need, and do follow-up work as needed, to make certain that client's needs are being met.</li>
<li>CAMS Intake: Provide intake evaluation, set service goals for client's time in program.</li>
</ul>

</ul>
<li>Case Management: Connect client with community services including but not limited to benefits, community case management, community counseling, alcohol and drug programs, vocational resources, medical resources, and ongoing medication. Works closely with PCC to create alternatives to psychiatric hospitalization.</li>
<li>Perform other job duties as assigned by your supervisor.</li>
Requirements for the Position
EXPERIENCE AND TRAINING
<li>Master's degree in psychology, social work, recreational therapy, music therapy, art therapy or a behavioral science field; OR</li>
<li>Bachelor's degree in nursing, or occupational therapy and licensed by the State of Oregon; AND</li>
<li>One year of postgraduate degree experience in the behavioral health field.</li>
PREFERENCES Preference will be awarded to candidates who are LCSW, CSWA or eligible for LCSW. Qualified candidates will be eligible for licensure supervision if hired. SPECIAL REQUIREMENTS
<ul>
<li>Must be either: 1) licensed by the Oregon Board of Licensed Professional Counselors, 2) licensed by the Oregon Board of Licensed Social Workers or 3) credentialed or registered through the Mental Health and Addiction Counseling Board of Oregon (MHACBO) as a Qualified Mental Health Professional (QMHP) within the first 30 days of employment.</li>

<li>Perform physical exams, diagnose and treat illnesses, order and interpret labs, prescribe medication, provide patient education, and appropriately document all relevant information in EMR.</li>
<li>Perform examinations for workers’ comp, occupational medicine, DOT, sports, wellness, and pre-employment screenings</li>
<li>Provide flexible and adaptive care so as to not have to turn away patients before exhausting reasonable treatment options.</li>
<li>Excellent communicator to both patients and staff.</li>
<li>Work autonomously as a provider and work alongside nurses, x-ray technicians, and front office specialists to provide the best possible patient care.</li>
<li>Provide leadership, direction, and assistance to team members as needed. Monitors completion of staff’s daily follow-up calls to monitor patient progress.</li>
<li>Ability to use judgement to determine when to consult supervising physician</li>
<li>Works with manager and other operational team members to ensure consistency with medical policy and professional standards.</li>
<li>Supports and engages in projects and activities which will develop or improve services delivered at the clinic.</li>
<li>Have a least one year of FNP experience and/or emergency care experience as an RN.</li>
